This extremely slim woman from Sichuan province in China, who is in her mid-20s and weighs just 42 kg, first came to Dr Arthur Tjandra of Elixir de Vie 1.5 years ago, after she had seen numerous plastic surgeons in Hong Kong, Guangzhou and Shenzhen, and was advised that it was impossible to harvest fat from such a slim person to perform autologous fat grafting breasts augmentation. She was offered breast implants by all of them, but she did not like the idea of putting foreign materials into her body. When Dr Arthur Tjandra of Elixir de Vie saw her, he agreed that she had extremely slim upper body, but she had extremely thick, fat thighs! Not only did she have thick, fat thighs, she also had flat buttocks and thick saddle bags which made her buttocks appeared droopy and sagging, a condition Dr Arthur Tjandra always refers to as: granny butts! There was also no angle between her buttocks and the back of her thighs because of the thick layer of fat in her lower buttocks as well as saddle bags. This made her appear as if she had no buttocks at all! Because her thighs were so thick, Dr Arthur Tjandra advised her to split her thighs liposuction into 4 separate sessions, half thigh each time, so that she could get 4 sessions of fat injection into her breasts. Although she has flat tummy, she has thick fat in her lower back/love handles, which can also be used to augment her breasts and/or buttocks.

She now returns to Dr Arthur Tjandra of Elixir de Vie to complete her thighs liposuction. During the first session, she had left outer half of thigh liposuction. During the second session, she had left inner half of thigh liposuction. During the third session, she had right outer half of thigh liposuction. This time round, she is getting liposuction done on the inner half of her right thigh. As she is very happy with the outcome of autologous fat grafting to her breasts, she is no longer getting another session of fat grafting to her breasts anymore.
The following video taken during this 4th visit to Dr Arthur Tjandra of Elixir de Vie shows the transformation of her bottom from a “butt-less” bottom to beautiful bubble buttocks now. The problem of “butt-less” bottom lay with the fact that she had thick fat layer over the bottom part of her buttocks and back of thighs. On normal people, men and women alike, there is usually an angle between the lower part of the buttocks/buttocks crease and the back of the thighs. The sharper the angle, the more acute the angle, the perkier the buttocks appear. In some people, instead of an angle, the buttocks slope down towards the back of thighs. This causes the appearance of sagging buttocks. In her case, it is even worse. Her buttocks and back of thighs appear as one continuous line, making her appear “butt-less”. Her “butt-less” butts are made worse by the complete lack of volume in the upper poles of her buttocks and presence of thick layer of fat over her lower back/love handles. With a combination of liposuction of her lower buttocks, saddle bags and back of thighs, as well as autologous fat grafting buttocks augmentation to her upper buttocks, which was done more than a year ago, she now has round beautiful bubble buttocks! When her love handles are eventually removed, the lower back is going to arch in even more, and gives the illusion of even much perkier buttocks!
This is good case study to show why it is extremely important to see the body as a 3-dimensional structure, and hence, perform liposuction is a 3-dimensional manner as well. Had she gone to see a surgeon who performed only standard inner and outer thighs liposuction, she would not have had such beautiful results! Unfortunately, circumferential or 3-dimensional liposuction carries the risk of deep vein thrombosis, which results in pulmonary embolism and eventually death! This risk is further increased if the surgery is performed under general anesthesia. This is a noteworthy complication which is well-documented in liposuction text book. This is why most surgeons refrain from offering and performing 3-dimensional/circumferential liposuction. However, after having performed more than 10,000 cases of 3-dimensional/circumferential liposuction using nothing but a syringe and micro-cannula with maximum diameter of 3 mm, Dr Arthur Tjandra of Elixir de Vie finds that syringe liposuction is extremely safe because the suction generated manually with a syringe is so low and the diameter of cannula is so small that they inflict minimal injuries as compared to machine-assisted liposuction. General anesthesia increases the risk of deep vein thrombosis and pulmonary embolism. That is why Dr Arthur Tjandra of Elixir de Vie only performs his liposuction surgeries under local anesthesia. The only drawback is syringe method leaves the surgeon with multiple repetitive strain injuries over his muscles, joints and tendons.
