This middle-aged woman comes to Dr Arthur Tjandra of Elixir de Vie with severe abdominal pain 1 week after abdominal liposuction by another plastic surgeon in Penang, Malaysia. The liposuction was performed with ultrasound method (VASER). Upon examination, Dr Arthur Tjandra of Elixir de Vie found complication of hematoma, which was collection of blood. It is understandable that with such traumatic way of performing liposuction, where the fat is firstly liquefied with ultrasonic heat, after which it is aspirated with powered suction device, excessive bleeding and swelling may occur. Unlike open surgery where bleeding points can be burnt with diathermy to stop bleeding, liposuction is a closed surgery where bleeding is stopped using compression garments only. In excessive trauma and bleeding, it may be impossible to stop bleeding with compression garments only. This is why hematoma occurs in much higher frequency with powered liposuction device. With syringe liposuction technique, Dr Arthur Tjandra of Elixir de Vie performs liposuction with only a 2 to 3 millimeter cannula and aspirate the fat with suction he generates with his own hands using only a plastic syringe. Such technique is especially damaging to the surgeon’s hands and arms, requiring such sheer determination, strength, stamina and endurance, however it is especially safe for patients. How much trauma can a 2-3 millimeter cannula and a plastic syringe inflict on your body? But, using such small cannula and a plastic syringe to aspirate liters and liters of fat out of your body is physically very challenging!

Unfortunately, some patients, misguided by internet reviews of other patients who have gone through liposuction with modern devices, think that liposuction should definitely cause a lot of bruising and swelling, and even hematoma, hence if such complications are not present, the surgeon must have cheated him/her!
